24/7 Voice Booking System — Gulf Healthcare Group

A Gulf clinic network with 12 locations was losing patients to after-hours voicemail. Reception staff spent 3–4 hours daily on booking calls. We replaced both with an Arabic/English voice AI receptionist running on the clinic's own servers.

Who gets the most from this

Different decision-makers care about different things. Here's what matters to each.

CEO / Owner

Every missed call after hours is a patient choosing the competitor down the street.

  • 340 calls/day automated = $94K saved annually in staffing
  • ROI in under 90 days at most clinic deployments
  • Competitive edge: most Gulf clinic networks still use voicemail
  • Fixed price — no budget blowout, no monthly SaaS fee
CTO / IT Director

Full source code. On your servers. No black box, no vendor dependency.

  • Sub-500ms Arabic voice AI with Deepgram Nova-3 + ElevenLabs
  • FastAPI websocket server — no polling, real-time streaming
  • Integrates with your existing EMR/PMS via REST or webhook
  • Patient data on-prem — you own every byte, full audit log
Operations Director

Your reception team should be focused on in-clinic patients, not the phone.

  • Handles booking, rescheduling, and cancellations end-to-end
  • SMS reminders cut no-show rates to near zero
  • Same system scales from 1 location to 20 with no rework
  • Complex calls route to staff with full context summary

The problems Gulf clinic networks share

Before building anything, we do a free 30-minute architecture call. These are the patterns we see across UAE and KSA clinic groups every time.

Calls dropped after hours

A patient calls at 9pm in Arabic — nobody picks up. The appointment goes to the competitor down the street.

Staff time on manual booking

Reception teams spend 3–4 hours daily confirming, rescheduling, and chasing no-shows instead of serving patients in clinic.

No-show rates running at 20–30%

Without automated reminders and easy rescheduling, appointment slots go empty while the waiting list grows.

Compliance and data anxiety

Patient data under UAE HAAD and Saudi MOH frameworks — the wrong vendor means liability. You need a system that keeps data inside your infrastructure.

What we build

A voice AI agent that runs your front desk after hours

Not an IVR menu. Not a chatbot. A real-time Arabic/English voice agent that holds a natural conversation, understands patient intent, and books the appointment — in under 30 seconds.

  • Sends Arabic and English SMS reminders 24h and 2h before each appointment
  • Handles rescheduling and cancellations without reception involvement
  • Routes complex calls (emergencies, specific doctor requests) to human staff
  • Full call transcript and booking log for compliance and audit trail
  • Integrates with existing practice management and EMR systems via API
  • Patient data processed on-premise — nothing leaves your server

Common questions

Is this compliant with UAE HAAD and Saudi MOH requirements?

Yes. We architect the system so all patient data — call recordings, booking records, transcripts — stays on your servers inside your infrastructure. No third-party cloud storage for PII. This approach aligns with UAE HAAD data protection standards and Saudi MOH digital health frameworks. We deliver a written architecture document before any code is written so your compliance team can review exactly how data flows.

Does it actually understand Gulf Arabic?

Yes. We use Deepgram Nova-3 which handles MSA and Gulf dialect (Khaleeji) natively — patients don't need to speak formally. ElevenLabs provides a natural Arabic voice response. We've run this in production for a Gulf clinic network with mixed Arabic dialect callers since June 2025.

What happens when a patient needs to speak to a real person?

The agent detects intent signals (urgency, specific doctor request, patient distress) and transfers the call to a live staff member, with a real-time summary of the call context so the staff member knows the situation before they pick up.

How does it connect to our current booking system?

Via REST API or webhook. We build a custom integration layer that reads your calendar availability and writes bookings directly into your existing practice management system — whether that's Clinicmaster, Practo, or a custom EMR. Setup takes 1–2 weeks of the build cycle.

What does the pricing include?

Everything: architecture design, engineering, testing, deployment on your infrastructure, 30-day post-launch support, and full source code handoff. You own the system. No monthly license fees. Voice infrastructure costs (Deepgram, ElevenLabs, Twilio) are usage-based at roughly $0.11/minute — a fraction of human agent costs.

Can we start with one location and scale?

Yes — the system is built to handle multiple locations from the start. Most clients deploy to one pilot location, validate over 2–4 weeks, then enable additional locations without rebuilding anything. Each location gets its own phone number routing and calendar integration.

You'll know what it costs and what it does before we write a line of code.
